VA forms you can file with VetEZ Claim

Official VA, Court and CRSC forms, with more being added. Answer plain-English questions, review the finished PDF, sign on screen, and send it to the right office. Nothing is sent until you confirm.

File a claim

Start a new claim, protect your effective date, or reopen a decided claim with new evidence.

  • VA Form 21-0966

    Intent to File

Tells VA you plan to file. It holds your effective date for up to one year while you gather evidence and finish the claim.

Usually the first form to send. Takes a few minutes.

    Faxed to VA

  • VA Form 21-526EZ

    New or increased disability claim

Claim a new condition, or ask VA to raise the rating for a condition it has already service-connected.

    Faxed to VA

  • VA Form 20-0995

    Supplemental Claim

Ask VA to look again at a decided claim because you have new and relevant evidence, or because a law changed.

Also used to reopen older claims.

Appeal a decision

You usually have one year from the date on your decision letter to choose a review lane.

  • VA Form 20-0996

    Higher-Level Review

Ask a more senior VA reviewer to look at the same evidence again for an error. No new evidence is allowed.

    Faxed to VA

  • VA Form 10182

    Board Appeal (Notice of Disagreement)

Take your case to a Veterans Law Judge at the Board of Veterans' Appeals. You choose direct review, more evidence, or a hearing.

Faxed to the Board of Veterans' Appeals

Appeal to the Court

If the Board of Veterans' Appeals denied you, you can appeal to the U.S. Court of Appeals for Veterans Claims within 120 days of the Board's decision.

  • Court Form 1

    Notice of Appeal

Starts your appeal at the Court. The app fills in the Court's current form and faxes it with a cover sheet.

    Faxed to the CourtWebsite only

  • Court Form 4a

Declaration of Financial Hardship (fee waiver)

Asks the Court to waive its $50 filing fee if paying it would be a hardship.

Statements and supporting forms

Forms that go with a claim or appeal: your own statement, a witness's statement, records releases and more.

  • VA Form 21-4138

    Statement in Support of Claim

Tell VA, in your own words, anything that is not on another form: what happened, how it affects you, or why a decision is wrong.

    Faxed to VA

  • VA Form 21-10210

Lay / witness statement (buddy statement)

A spouse, friend or someone you served with describes what they saw. You send them a secure link; they fill it in and sign.

Available on the website now; coming to the phone app.

Filled in by your witness, then faxed to VA

  • VA Form 21-0781

Statement in support of claim for PTSD and other mental health conditions

Describe the in-service events (stressors), including combat or personal trauma, behind a mental health claim.

    Faxed to VA

  • VA Form 21-4142 and 21-4142a

    Medical records release

Let VA get your records from private doctors and hospitals.

    Faxed to VA

  • VA Form 21-8940

    Individual Unemployability (TDIU)

Ask to be paid at the 100% rate because service-connected conditions keep you from holding a steady job.

    Faxed to VA

  • VA Form 20-10206

Request your claims file (C-file) and DD-214

Ask VA for a copy of your records under the Freedom of Information Act or Privacy Act.

Military retirees

Combat-Related Special Compensation is decided by your military service, not VA.

  • DD Form 2860

    Combat-Related Special Compensation (CRSC)

Apply for tax-free CRSC pay if you are a military retiree with VA-rated disabilities that are combat-related.

E-mailed to the Army, Navy/Marine Corps or Coast Guard CRSC office. For the Air Force and Space Force, NOAA Corps and Public Health Service, you get a copy to print and mail.

E-mailed to your service's CRSC office

How sending works

The app fills in the current official form with your answers. You open the finished PDF, check it and sign on screen. When you tap send, the form is faxed to the office that takes it: VA's claims intake for claims and supporting forms, the Board of Veterans' Appeals for a Board Appeal, and the U.S. Court of Appeals for Veterans Claims for a Notice of Appeal. CRSC applications are e-mailed to your military service's CRSC office instead, because CRSC is decided by your service, not VA.

You get a confirmation number right away, and the History screen lets you check delivery later. Prefer to upload on VA.gov yourself? Use Review PDF to save the finished form, then upload it there.

Deadlines the app reminds you about

  • An Intent to File protects your effective date for one year.
  • You generally have one year from the date on a VA decision letter to file a Higher-Level Review, Supplemental Claim or Board Appeal.
  • You have 120 days from a Board decision to file a Notice of Appeal with the Court.

These are the official forms, free from VA at va.gov/find-forms. VetEZ Claim is a self-help tool, not legal advice, and using it does not appoint a representative. VA-accredited representatives help veterans for free: find one on VA.gov or see more ways to get help. More answers are on our FAQ.
